Waiver This document must be signed by each person and in the case of minors, by the parents of each person who intends to participate in activities Base Camp Miami/Grove Skatepark By signing this agreement, you agree that you will not make demand upon or sue the operator and owner of the Base Camp Miami/Grove Skatepark or the City of Miami, and you agree to Indemnify the Owner and Operator of the skate park, City of Miami and save them harmless from any damages that they may incur resulting from your use of the skate park. Recitals:

1. The undersigned understands, acknowledges and agrees that in-line skating, skateboarding, and other wheel-based activities, as are permitted at Base Camp Miami/Grove Skatepark, are dangerous activities.
2. Such activities can result in injury to the person or damage to property and other.
3. The undersigned is fully aware of the risks inherent in such activities;
4. The undersigned is aware of the conditions at Base Camp Miami and is aware that those conditions may become more dangerous during the time that the undersigned is using Base Camp Miami;
5. The Undersigned acknowledges that he or she is assuming all risk of loss, damage or injury while on the premises of Base Camp Miami
6. the undersigned acknowledges that there are various levels of skills and experience required for the use of the different skate boarding / wheel based activity surfaces at Base Camp Miami and the member will abide by the rules and regulations for the use of Base Camp Miami;

After acknowledgment of the foregoing recitals and in consideration of the right to use the premises at Base Camp Miami, upon the payment of such fees as from time to time as may be established by Base Camp Miami., the undersigned hereby releases Base Camp Miami,, the Operator of Base Camp Miami, The City of Miami the owner of the real estate where the Base Camp Miami is located, and their administrators, executors, heirs, successors, assigns and legal representatives, from and assumes the risk of any and all liability for bodily injury, death or property damage to the undersigned, or rising out of the undersigned's use of, or presence at Base Camp Miami, whether or not the injuries or damages were caused by the negligence of Base Camp Miami or others. The benefit of this release extends to Base Camp Miami their subsidiaries, affiliates, partners, officers, directors, employees and agents, and each of their respective executors, administrators, heirs, successors, assigns, and local representatives. The undersigned agrees not to sue any and all of such persons and entities in connections with any and all damage of any nature, all injuries to the person or damage to the property of the undersigned, save and excepting only such injuries and/or damages as may result from the gross negligence or willful misconduct of Base Camp Miami. The undersigned (or if this agreement is signed by a Parent or Guardian of a minor, the undersigned's parent or guardian) agrees to indemnify and save harmless, Base Camp Miami,City of Miami . their partners, affiliates, officers, directors, employees and agents, and their executors, administrators, heirs, successors, assigns, and legal representatives from any and all damages, actions, causes of actions, claims, judgments, costs, and attorney's fees which may in anyway and at any time result from the members use of or presence at Base Camp Miami, including damage to the property of Base Camp Miami or One Cool World Inc. APPEARANCE RELEASE

For good and valuable consideration, receipt of which is hereby acknowledged, I authorize Base Camp Miami (“Producer”) and their respective parents, affiliates, subsidiaries, licensees, successors and assigns to make use of my appearance for the program (“Program”) and in connection with Producer and/or Company or otherwise. I agree that you may tape and photograph me, and record my voice, conversation and sounds, including any performance of any musical composition(s), during and in connection with my appearance and that you shall be the exclusive owner of the results and proceeds of such taping, photography and recording with the right, throughout the world, an unlimited number of times in perpetuity, to copyright, to use and to license others to use, in any manner, all or any portion thereof or of a reproduction thereof in connection with the Program or otherwise. For purposes of clarity, I expressly waive any and all moral rights I may have in connection with my appearance. I further agree that you may use and license others to use my name, voice, likeness and any biographical material concerning me which I may provide, in any and all media and in the promotion, advertising, sale, publicizing and exploitation of the Program and/or otherwise and ancillary products (e.g., merchandise) in connection with the Program and in connection with Producer and/or Company or Company's affiliated services, throughout the world in all media, an unlimited number of times in perpetuity. I further represent that any statements made by me during my appearance are true, to the best of my knowledge, and that neither they nor my appearance will violate or infringe upon the rights of any third party.I hereby waive any right of inspection or approval of my appearance or the uses to which such appearance may be put. I acknowledge that you will rely on this permission potentially, at substantial cost to you and hereby agree not to assert any claim of any nature whatsoever against anyone relating to the exercise of the permissions granted hereunder.

All information disclosed to me relating to the clients, cast, themes, plots, story and story elements, designs, effects and special effects, hardware, artwork and visual representations in connection with the development and production of the Series shall be considered confidential and shall be retained in confidence by me. This includes talking to and with any press media regarding any of the above. Any breach relating to this Non-Disclosure Clause can be cause for immediate dismissal and can be cause for Producer and Company to seek monetary damages at their discretion.
